The role of the Social Services department is to work with Allina Health United Hospital – Hastings Regina Campus patients in planning and accessing services.
Services before, during, and after a hospital visit
- Assist with transition from one environment to another
- Coordinate referrals and identify resources for:
- Home Health Care services
(nurse visits, home health aid, homemaker)
- Extended care services
(skilled nursing facilities, assisted living, adult foster care)
- Senior services
(Meals on Wheels, Senior Center, caregiver respite, lifeline)
- Financial assistance eligibility for programs
(Medicare, Medical Assistance, and other financial resources)
- Home medical equipment
(oxygen, hospital beds, wheelchairs, etc.)
- Act as a contact for the patient/family regarding patient health issues
- Act as a liaison to the Ethics Committee
- Provide information on Health Care Directives
Senior Living residents, families, and community members
The Social Services staff also provides assistance to Senior Living residents, families, and community members in the areas of:
- Counseling and support
- Crisis intervention
- Outreach services
- Information and referrals
- Psychosocial assessments
- Emotional concerns
- Education
- Support groups
- Safe place for newborns
