The Allina Health Ambulatory Care Pharmacy PGY1 Residency provides an opportunity for pharmacists to further their clinical, practice management and teaching skills in a warm, supportive environment. Residents will be able to partner with patients to improve health in a variety of disease states by providing Comprehensive Medication Management. Allina Health’s residency program is a member of the University of Minnesota’s larger PGY1 multisite program providing residents with group learning opportunities, teaching certificates, research project assistance and the support of co-residents, among other benefits.
Allina currently has 15 clinical pharmacists providing comprehensive medication management services to patients. These pharmacists see patients in 27 clinic locations and work under a robust Collaborative Practice Agreement in coordination with providers. Visits occur in primary care clinic offices, over the phone, virtually and in the patient’s home. Allina pharmacists are an integral part of the care management program, which is an interdisciplinary team that includes registered nurses, licensed social workers and non-clinical care guides. The team’s focus is to improve transitions of care and reduce hospital readmissions. Additionally, some of the pharmacists are team members in the Allina Health Senior Health Co-Care Program. This home-based program provides primary care in the home geriatric patients.
Allina Health is a not-for-profit health care system that cares for patients from beginning to end-of-life through its 90+ clinics, 12 hospital campuses, 14 retail pharmacies, and 52 rehabilitation locations. We provide services in the Twin Cities metro area and various greater Minnesota and Wisconsin locations. Residents will spend a majority of their time caring for patients at Allina Health Shakopee and Plymouth Clinics. The resident will also work with a Senior Health Co-Care team that sees patients in the northwest metro region.
