2023-2025 needs assessments and implementation plans

During 2021–2022, Allina Health worked with local public health and communities surrounding each of its hospitals to assess community health, determine priorities and develop a plan to address those priorities the next three years.

Four priorities emerged throughout our system:

• mental health and wellness
• substance use prevention and recovery
• social determinants of health and health-related social needs
• access to culturally responsive care.

Below are links to each hospital's report and executive summary, including:

• a description of the hospital and the demographics of the county/communities surrounding the hospital
• the detailed process followed to review local data and gather input from a variety of stakeholders and the results of the process
• the implementation plan with our goals, strategies and activities for 2023–2025.
